I recognise this so clearly in two family members it is scary! You've broken this down so well and it speaka to exactly what I see going on. Getting them both to see is quite another thing. I've tried numerous times to no avail...

When this happens to me, it feels like my brain is having its own internal conflict.

I can recognize when my threat response kicks in and I'll even tell myself to stop talking. But the urge to say something and keep engaging is so strong that it feels like I can't help myself. Even in the back of my mind, I know I'm probably going to regret saying something.

Having awareness alone isn't always enough for me to stop the conversation from escalating. This is definitely an area I need to do better in.
